You're the hub of the care team. You'll work alongside Primary Care Providers, Medical Directors, and Medical Assistants to deliver coordinated, high-quality primary care to residents of senior living communities across California, Nevada, and Arizona — all from your home office. If you're a clinically sharp RN who communicates well, takes ownership, and wants to do meaningful work without a commute, this role was built for you. What You'll Do <ul><li>Collaborate with a dedicated team, supporting a substantial patient panel alongside Primary Care Providers (PCPs).</li><li>Triage inbound communications, including calls, faxes, email and instant messages from teammates, patients and partners.
