What You Will Do  - Essential Functions

The registered nurse will be based in the FL market. With responsibility for supporting the patient panel attributed to their care team pod, in partnership with their Advanced Practice Provider and Community Health Worker this individual will have specific responsibility for both direct care delivery and care coordination.

  • Clinical Care Management & Monitoring
    • Conducts intake/enrollment visits and ongoing telephonic nursing assessments, including routine, urgent, post-hospital, and risk-based care calls to monitor chronic and acute conditions.
    • Identifies changes in patient condition, manages triage needs, facilitates medication reviews and reconciliations, and supports implementation of provider medical orders.
  • Care Coordination & Collaboration
    • Collaborates closely with Lucet providers, internal care teams, and external partners (PCPs, specialists, home care, pharmacy, caregivers) to ensure coordinated, comprehensive care.
    • Proactively manages visit scheduling, reviews provider schedules and patient visit history, and communicates care changes across the clinical team through daily provider touchpoints.
  • Documentation, Education & Quality Outcomes
    • Develops and maintains integrated care plans addressing patient goals, barriers, interventions, and self-management strategies while ensuring all encounters are documented per standards.
    • Provides patient education and resources, addresses preventive and quality measures, closes care gaps, and sustains progress to improve long-term health outcomes.

Who You Are

Required Qualifications:

  • Holds active, unencumbered license in state of practice
  • Experience in care of adult, chronically ill patients
  • Triage experience preferred
  • Two or more years of practice experience required
  • Two or more years of home health or community healthcare experience preferred
  • Driver’s license without restrictions and good driving record
  • Mostly remote with occasional travel, up to 10% travel time
  • Bilingual preferred

Competencies

  • Care Coordination and Communication: The RN is responsible for completing intake/enrollment visits, conducting routine telephonic nursing care calls, and maintaining regular contact with the internal care team, participants, community providers, and other authorized contacts or caregivers. The RN coordinates care for participants with both internal and external clinical care team members (PCP, specialists, home care, case managers, pharmacies, etc.). The RN triages patient calls and escalates to the provider when needed.
  • Care Plan Development and Management: The RN collaborates with the care team to develop, review, and update integrated nursing care plans, implements, updates and reviews interventions to meet care goals.
  • Patient Education and Chronic Condition Monitoring: The RN provides effective patient education, monitors chronic conditions to minimize exacerbations, and facilitates medication reviews, reconciliations, and clinical orders.

Working conditions:

  • High-speed internet service (cable or fiber optic) with minimum download Speed of 20 Mbps, Upload Speed of 5 Mbps, and Maximum Latency of 100 milliseconds (must be installed before starting) required.
  • Frequent use of computer and phone systems
  • Must be able to constantly remain in a stationary, sitting position, communicate and exchange information with others, inspect information, perform repetitive motions with arms and fingers, interpret data, problem solve, make decisions, organize and plan, and maintain a positive and professional attitude in all situations
  • Work is performed from home with company-provided equipment. Sitting for long periods of time is expected and use of fingers and hands for typing is necessary.
  • A quiet workspace with minimal background noise for calls.
